Student Registration with school supplies

Melrose is about community and we are pleased that you have chosen to join the other parents, families, and students that make up the Melrose Public Schools educational community.

  Registration Information for the 2023-2024 School Year    

*Registration is required for all new students and families.

 Please carefully review the registration information provided below.

 KINDERGARTEN REGISTRATION INFORMATION:

  • Student must be 5 years of age by September 1, 2023
  • Student must be a Melrose Resident 

Step 1  

Pre-Registration: 

Use the links below to complete the required online  pre-registration paperwork for your child.

Step 2 

In Person Scheduled Appointment:

 At the end of the online form, you will be asked to schedule an in person appointment to complete your registration.  We ask that only one parent or guardian attend the appointment, children are not required to attend.     

Step 3 

Gather the documents needed for your in person appointment.  The following documents are required to complete registration:

  • Birth Certificate:  Please bring an original copy of your child's birth certificate with seal, we will make a photocopy.
  • Parent/Guardian's License or Passport:  We can make a photocopy. 
  • Most Recent Physical and Immunizations: Please bring in the most recent information within the last 12 months.  
  • Proof of Residency:  1 Document from this category, such as:  Recent Mortgage Statement Paid, Copy of your Purchase and Sale, Lease Contract, Section 8 Agreement, Tax Bill, or Tenancy at Will.  Documents must be fully signed. Documents must have your name and address on them. 
  • Proof of Occupancy:  2 Documents from this category, such as: Gas/Oil Bill, Home Phone Bill (not cell), Cable Bill, Water Bill, Health Insurance Benefits, these two documents must have your name and address on them.
  • Release of Student Records Form: Please fill out this form and bring with you.   This is only needed for students entering grades 1-12. 
  • If you have any student records such as report cards/student records, please bring those as well.  

All Scheduled In Person Appointments are held at: The Central Administration Office, 360 Lynn Fells Parkway, Melrose. (the entrance to Central Administration is located in the courtyard separating the high school and middle school)
